APPOINTMENT INFORMATION
Before your visit:
Once your appointment is confirmed, you will receive an email with links to our intake and consent forms. Please bring these with you, submit them online before your visit, or arrive 15 minutes early to complete them. If you have xray or MRI reports, or relevent lab testing results, you can submit these online or bring them in with you.

Please make SURE TO EAT lightly before your visit.

Wear loose fitting clothing that will allow access to your knees and arms.

Please avoid wearing lotions and perfumes, as many of our patients are fragrance sensitive.

Please arrive 5 minutes before your visit. We will do our best to accommodate late arrivals but please note your visit may be shortened.
Payment: Cash, checks, or credit cards are accepted at time of service. Payment in full is required until insurance benefits are verified.
Cancellation Policy: We ask that you provide us at least 24 hours notice if you need to cancel your appointment. This allows us to offer the appointment slot to another patient. Thank you for your mindfulness!
INSURANCE COVERAGE
Acupuncture: I am in-network for acupuncture with United Health Care. I am also in-network for American Specialty Health (ASH) plans, including Aetna, Anthem, Kaiser, Scan, and some Blue Shield of California plans, but am not able to accept new acupuncture patients with ASH plans at this time.
Chiropractic: I am in-network with United Health Care. Medicare provides some benefits for chiropractic care.
Manual therapies: Craniosacral Therapy, Visceral Manipulation and structural bodywork are generally not specifically covered by insurance plans. However, if they are performed in conjunction with chiropractic care or acupuncture treatment, I may be able to bill insurance for those portions of care that are covered.
Insurance coverage varies widely, and I am happy to check your benefits and discuss coverage. To do this, I will need a photo of the front and back of your insurance card, as well as your date of birth.
